It is an honor to be part of Belmont High School athletics. In order to have the opportunity to join the JV Cheerleading Team this Summer, there are a few more steps you need to complete. 1. On the home page of this site, you will find a tab labeled “files & links”. Print the emergency card. Have your parents fill out the information completely. 2. Make one copy of your medical insurance card both front and back. 3. Upload a picture of your medical insurance card to your phone or computer, whichever you will use for the next step. 4. Log onto www.AthleticClearance.com and complete online athletic clearance. You can click on Cheerleading photos for a step-by-step guide to the online clearance. This will take about 20 minutes and will require your parents to e-sign as well. 5. On the tab labeled “files & links” you will also find a Physical Form that you must print and take to your doctor. Physical appointments take a while to be granted, I suggest you try to make an appointment with your doctor today or tomorrow. Before you take this form to the doctor you need a few things: * Fill out the front page * If you wear glasses make sure you take them to your physical exam * If you have asthma please go to “files & links” and print the Asthma Action Plan sheet and take with you to your appointment. Click on Cheerleading photos to see important part of the physical forms that need to be completed. Your emergency card, medical insurance copy, and physical form will be due at the Parent Meeting.
